Diploma/Degree Program (Level Two) Forms

Application for Admission For first-time applicants or students that have been inactive for two years or more.
 Enrollment Form Each student must fill out and sign this form for each course.
 INSTE Online: Enrollment Form This form must be filled out and signed every time an INSTE Online student enrolls in a new course.
 Book Order Form A book order form must be submitted for each group along with payment and an application and/or enrollment form for each student.
 Beyond Academics Manual This manual explains the function and goals of this important component of a student’s formation.
 Leader Evaluation of Course This evaluation will be filled out for each course.
 Mentor Evaluation of Student This evaluation will be filled out for each course.
 Audit A Course Only one person per group may audit. Any exception must be approved by the Office of the Registrar.

Intent to Graduate Form (submit online)

Intent to Graduate Form 

 All candidates for graduation must complete this form whether or not they are able to attend the graduation ceremony.
 Mentor Data Sheet This form accompanies the student’s application.
 Student Evaluation of Course This evaluation will be filled out for each course.
 Transcript Request Form Any request for a transcript must be made using this form.
Application Upgrade to a Degree Level Two Diploma graduates with the requisite transfer credit may apply to upgrade to the degree.
 Review A Course An INSTE graduate (Level One or Level Two) may review a course previously studied without cost provided there is room in the group.
 Petition for Reexamination For permission to retake the final exam this form must be filled out and submitted.
 Scholarship Assistance Request Form This form must be filled out by the student and the group leader.
 Degree Student Guide Describes the reading and writing assignments that are required for all Degree students.
Petition for Time Extension Submit this form in order to request a time extension of three months. Each student in the group must submit this form.
