Certificate Program (Level One) Forms

Application/Enrollment Form This form must be filled out and signed for each course by each student.
INSTE ONLINE: Application /Enrollment form This form must be filled out and signed for each course. Please note a pastor or church leader must sign this form in addition to the student. ONLINE STUDENTS ONLY.
Book Order Form A form for each group must be submitted with payment plus an application / enrollment form for each student.
Evaluation of Participation This evaluation will be filled out each semester.
Certificate Request Form The submission of this form is necessary to receive the Certificate in Christian Ministry.
Leader Evaluation of Course This evaluation will be filled out for each course.
Audit A Course Only one person per group may audit. Any exception must be approved by the Office of the Registrar.
Student Evaluation of Course This evaluation will be filled out for each course.
Transcript Request Form Any request for a transcript must be made in writing using this form.
Review A Course An INSTE graduate (Level One or Level Two) may review a course previously studied without cost provided there is room in the group.
Petition for Reexamination For permission to retake the final exam this form must be filled out.
Youth Upgrade to College Credit Form for upgrading from special audit status to regular student status.
Scholarship Assistance Request Form This form must be filled out by the student and the group leader.
Petition for Time Extension Submit this form in order to request a time extension of six months. Each student in the group must submit this form.
